The Hospice Registered Nurse plays a crucial role in delivering compassionate care to patients in their homes. This position involves planning, organizing, and directing hospice services, with a strong focus on community health education and nursing expertise.
Key Responsibilities:
- Conduct comprehensive assessments of patients and their families to identify hospice care needs.
- Utilize the nursing process to provide professional nursing care.
- Develop and implement individualized care plans based on patient assessments.
- Adjust care plans as necessary to meet changing patient conditions.
- Engage patients and families in the care planning process.
- Administer prescribed medications and treatments in accordance with physician directives.
- Provide health education and counseling to patients and families.
- Assist patients with daily living activities to promote comfort and independence.
- Coordinate care with interdisciplinary teams to ensure continuity of services.
- Maintain accurate clinical documentation regarding patient care and progress.
- Participate in quality improvement initiatives to enhance patient care.
- Graduate of an accredited nursing program.
- Bachelor's degree preferred, with at least one year of hospice experience.
- Supervisory experience with hospice aides is advantageous.
