About the Role

We are seeking a highly organized and detail-oriented Project Coordinator Assistant to support the Special Projects Coordinator in various initiatives. This is a full-time, hourly position with core hours of 8:30 am - 4:00 pm, with occasional evenings and weekends as needed.

Key Responsibilities
  • Private Pay Home Delivered Meals
    • Engage with referral sources and process referrals
    • Support delivery routing
    • Support monthly re-enrollment of Private Pay clients
    • Communicate with eligible clients regarding intake
    • Organize frozen meal boxes for delivery
  • Maternal Health Pilot Project
    • Engage with referral sources and process referrals
    • Support client intake
    • Organize frozen meal boxes for delivery
    • Deliver meals (when needed)
  • Community Home-Delivered Meals
    • Engage with referral sources and process referrals
    • Support client intake
    • Organize frozen meal boxes for delivery
    • Deliver meals (when needed)
  • Community Health Work (CHW)
    • Assess client intake to determine highest need for CHW support
    • Provide CHW support to clients on Medicaid
    • Other duties, per the ever-changing needs of the organization

    • Excellent written communication skills
    • Excellent organizational skills
    • Proficiency in Microsoft Word, Excel, PowerPoint
    • Working knowledge of Microsoft TEAMS
    • Valid driver's license and reliable transportation
  • Education and Experience
    • 1-3 years of experience in a nonprofit setting
    • High School degree or equivalent education or experience
    • Experience working with diverse populations
    • Clear results of a background and motor vehicle record check
    • Community Health Worker Certification, preferred
    • Spanish Language Proficiency, preferred
Work Environment

Work is performed in a building. The noise level in the work environment is usually moderate and the environment is sometimes fast-paced. Protocol for safety and security is a priority. Occasional work after normal business hours. Must be able to drive to other locations such as external meetings, within the state.

Physical Demands

While performing the duties of this job, the team member is regularly required to stand; walk; sit; use hands to handle, or feel; reach with hands; balance, stoop, kneel or crouch; and talk or hear. The team member must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.
