Key Responsibilities
  • Facilitates and oversees the management of files and documentation within the department.
  • Actively engages in Case Management and Treatment Team discussions.
  • Reviews patient records to ensure compliance with admission and treatment continuation criteria; collects data and responds to requests for records from fiscal intermediaries; compiles clinical and financial information and communicates the status of both active and closed accounts for various levels of Case Management reporting.
  • Demonstrates the ability to work autonomously while exercising sound judgment.
  • Possesses knowledge of Federal, State, and intermediary regulations pertaining to inpatient acute care and lower levels of care.
  • Coordinates communication regarding discharge referrals as requested by clinical personnel, fiscal intermediaries, patients, and their families.
Qualifications

EDUCATION, EXPERIENCE, AND TRAINING REQUIREMENTS

1. An Associate's or Bachelor's degree in healthcare or a related field, or equivalent professional experience.

2. Prior experience in an acute care setting is preferred.

3. Proficient in intermediate computer skills, including data entry.

4. Excellent verbal and written communication skills.
