Key Responsibilities:
- Conduct thorough assessments to identify and prioritize the clinical and psychosocial needs of clients.
- Develop personalized care plans in collaboration with clients, their families, and healthcare providers.
- Implement and monitor care plans, ensuring effective communication among all parties involved.
- Advocate for clients, promoting self-management and access to necessary healthcare resources.

Qualifications:
Required:
- Active RN license in your state of residence.
- 3+ years of clinical nursing experience.
- Strong understanding of case management principles and patient-centered care.
- Excellent communication and organizational skills.
Preferred:
- Prior experience in telephonic case management.
- Case Management Certification (CCM) is a plus.
- Ability to work independently in a remote environment.
