Responsibilities
  • Facilitates and oversees the flow of documentation within the department.
  • Actively engages in Case Management and Treatment Team discussions.
  • Reviews patient records to ensure compliance with admission and treatment criteria; collects necessary data and responds to record requests from fiscal intermediaries; compiles clinical and financial information while updating the status of accounts for Case Management reporting.
  • Demonstrates the ability to work autonomously and exercise sound judgment.
  • Possesses knowledge of federal, state, and intermediary regulations related to inpatient care and various levels of healthcare services.
  • Coordinates discharge referrals as needed, communicating effectively with clinical staff, fiscal intermediaries, patients, and families.
Qualifications

EDUCATION, EXPERIENCE, TRAINING

1. An Associate's or Bachelor's degree in healthcare or a related field, or equivalent experience in a relevant area.

2. Preferred experience in acute care settings.

3. Proficient in computer skills, including data entry.

4. Strong written and verbal communication skills.
