Job Title: Waiver Program Coordinator

Join the Massachusetts Rehabilitation Commission as a Waiver Program Coordinator and play a vital role in supporting individuals with disabilities to live independently in their communities.

About the Role:

We are seeking a highly motivated and organized individual to coordinate statewide waiver services for individuals with Acquired Brain Injury and Money Follows the Person - Community Living Waiver participants. As a Waiver Program Coordinator, you will be responsible for needs assessment, service plan development, service authorization, and implementation and monitoring of individualized service plans (ISPs).

Key Responsibilities:
  • Engage with individuals, their guardians, and families to develop person-centered plans that meet their unique needs and goals.
  • Coordinate interdisciplinary teams to develop and implement ISPs, ensuring seamless service delivery and support.
  • Partner with Adaptive Services Coordinators to ensure home accessibility modifications meet individual needs and are completed in a timely manner.
  • Conduct outreach in nursing facilities and rehabilitation hospitals to educate staff about Home and Community-Based Waivers.
  • Collaborate with clinicians to assess and determine strategies for addressing individualized needs.
Requirements:
  • At least three years of full-time or equivalent part-time experience in program management, administration, or coordination.
  • Excellent communication and problem-solving skills, with ability to work comfortably with remote technology.
  • Familiarity with the needs of individuals with disabilities living in the community.
Preferred Qualifications:
  • Knowledge and experience working with individuals with disabilities and their families, preferably those with Acquired Brain Injury.
  • Knowledge of Medicaid, state agencies, and community-based resources available to serve persons with disabilities or elders.
