Position Overview

SUMMARY

The South Middlesex Opportunity's Young Parent Living Program is dedicated to supporting single young parents aged 14-22 and their children. The Case Manager plays a crucial role in guiding families through the development and execution of individualized service plans, educational initiatives, and group activities aimed at fostering personal growth and stability.

KEY RESPONSIBILITIES

  • Facilitate access to essential services, assist in goal-setting, and create actionable plans for families.
  • Mediate conflicts and challenges that arise between families and their adolescent members.
  • Demonstrate effective parenting techniques in both structured settings and informal interactions.
  • Ensure adherence to established program protocols.
  • Collaborate with external agencies to secure long-term resources such as childcare, education, healthcare, and counseling.
  • Maintain thorough and accurate documentation for each family served.
  • Engage in self-assessment to identify and pursue professional development goals.
  • Contribute to a safe and clean environment within the program.
  • Possess a valid driver's license to provide necessary transportation for clients.
  • Conduct family intake assessments.
  • Participate in on-call duties as required.
  • Enhance relationships with key resources within the client support network.
  • Attend and contribute to staff and house meetings.
  • Complete mandatory training sessions as required by the agency.
  • Engage clients by understanding and addressing their needs, whether within or outside the scope of the program.
  • Participate in engagement team meetings, follow up on client service plans, and communicate effectively with clients and colleagues across different specialties.
  • Ensure compliance with program, department, agency, and funding requirements, as well as organizational policies and procedures.
  • Perform additional duties as assigned.

QUALIFICATIONS AND SKILLS

  • Experience working with homeless individuals and/or adolescents is preferred.
  • Excellent verbal and written communication abilities.
  • Bi-lingual candidates are preferred.
  • A Bachelor's degree in counseling, social work, psychology, or a related field is required, or equivalent experience as a paraprofessional with a minimum of three years in the field.
  • A valid driver's license is essential.

ORGANIZATIONAL STRUCTURE

  • This position reports directly to the Program Director.
  • There are no direct reports associated with this role.

WORK ENVIRONMENT

The program operates from a multi-story residence, and staff are expected to conduct regular checks throughout the facility to address the needs of guests. The role involves minimal physical lifting, but driving is a necessary component, as the program is responsible for transporting all referrals. As part of this position, the Case Manager will have direct or incidental interactions with clients served by South Middlesex Opportunity across various programs funded or managed through the Executive Office of Health and Human Services. A background check is required.
