Job Overview

Position: Clinical Case Manager

Work Arrangement: Hybrid, Monday to Friday (8:30am-5:00pm)

About the Role:

We are looking for a compassionate and skilled Clinical Case Manager to join our dedicated team at Heartland Human Care Services. In this role, you will provide essential clinical case management services to a diverse range of individuals, including vulnerable populations such as children, adults affected by trafficking, families, immigrants, refugees, and those facing mental health challenges, HIV/AIDS, or substance use disorders.

Key Responsibilities:

  • Deliver comprehensive clinical case management through a short-term model, including advocacy, crisis intervention, and referrals to internal and external resources.
  • Engage regularly with assigned cases and community representatives, developing and monitoring individualized treatment plans to ensure achievable goals are met.
  • Conduct outreach and intake processes, creating tailored care plans and tracking progress while adhering to funder requirements.
  • Establish service plans that outline actionable steps for each participant, advocating for their access to benefits and entitlements.
  • Maintain accurate and timely documentation, including case recordings and statistical reports.
  • Foster professional relationships with social service, health, and governmental agencies, ensuring compliance with confidentiality laws and clinical standards.
  • Participate in supervision meetings, team discussions, and training sessions to contribute to program planning and operational improvements.
  • Uphold a high volume of work in alignment with project standards, integrating strengths-based approaches in clinical practice.
  • Adhere to professional standards as outlined by relevant governing bodies and participate in ongoing training to ensure compliance.
  • Perform additional duties as assigned.

Qualifications:

  • Bachelor's degree in Social Work or related field with three years of experience, or a Master's degree in Social Work, Counseling, or Clinical Psychology.
  • Fluency in Spanish is highly preferred.

Certifications:

  • Possession of an LPC/LSW license may be required; certification as a Domestic Violence Professional is preferred.
  • A valid driver's license may be necessary.

Skills and Abilities:

  • Strong communication skills, with the ability to engage effectively with diverse populations.
  • Proficiency in Microsoft Office and familiarity with various software applications.
  • Ability to work independently while managing multiple priorities.
  • Demonstrated emotional intelligence and analytical skills.

Work Environment:

The work environment is typically moderate in noise level, with program sites located throughout the community. Flexibility in work location is required, with expectations for in-person meetings as necessary.

Physical Requirements:

  • Regularly required to communicate verbally and in writing.
  • Ability to sit, stand, walk, and occasionally lift up to 25 pounds.
