We are seeking a dedicated Pediatric Clinical Care Coordinator (RN) to join our outpatient unit.

Position Overview:
We are currently looking for a full-time Pediatric Clinical Care Coordinator (RN) to support our outpatient services.

Our facility specializes in the acute care of children with intricate medical issues, chronic conditions, developmental disabilities, and trauma.


The Pediatric Clinical Care Coordinator (RN) will work closely with an assigned group of patients and their families, overseeing their health management plans.

This role involves actively addressing the clinical needs of patients in collaboration with the aligned physician within the outpatient setting.

The Pediatric Clinical Care Coordinator (RN) will utilize critical thinking and nursing expertise to ensure effective clinical and episodic care coordination, while also meeting quality-related benchmarks.


Key Responsibilities:

  • Assist physicians in all facets of patient consultations, treatment planning, and follow-up care.
  • Act as a liaison for families, care coordination entities, and external organizations.
  • Leverage pediatric knowledge and skills to coordinate comprehensive care for pediatric patients across the healthcare spectrum.
  • Manage prior authorization requests in compliance with payer guidelines for prescribed medical equipment and supplies.
  • Provide education and support to patients and families regarding the prescribed treatment plans.

Qualifications:

  • Graduate of an accredited nursing program; BSN required, MSN preferred.
  • A minimum of three years of pediatric nursing experience is essential.
  • Experience in care management or a clinical specialty is advantageous.
  • Current Illinois Professional Nursing License; APN license if applicable. Certification in Pediatric Nursing, Case Management, or a relevant specialty is preferred.
  • Strong written and verbal communication skills, along with excellent interpersonal abilities and expertise in pediatrics and family education.
  • CPR certification required: Only certifications from the American Heart Association or the American Red Cross will be accepted.
