Role SummaryThe Wound/Ostomy Nurse Clinician Coordinator embodies the mission, vision, and values of Northwestern Memorial Healthcare, adhering to the organization's ethical standards and compliance protocols. This role operates within a collaborative, interprofessional setting, necessitating outstanding interpersonal abilities to cultivate and maintain effective relationships with patients, healthcare providers, and support teams.
Key Responsibilities- Deliver evidence-based wound and ostomy care to patients.
- Conduct thorough assessments of wounds and provide expert guidance on etiology and staging of pressure injuries.
- Support the healthcare team in implementing prevention and treatment strategies for wound-related issues.
- Perform ostomy assessments and markings for both new and existing ostomies, ensuring comprehensive post-operative care and education.
- Engage in prevalence surveys and ongoing monitoring of pressure injuries.
- Identify trends in nursing practices and collaborate with leadership to enhance quality patient care focused on pressure injury prevention.
- Analyze pressure injury data to establish expected outcomes and contribute to the development of individualized care plans.
- Coordinate care delivery and assess progress towards achieving patient outcomes.
- Utilize the nursing process to perform comprehensive patient assessments and propose solutions to the interprofessional team.
- Recognize and address abnormalities detected during assessments and procedures.
- Collaborate with clinical services and external agencies to ensure coordinated patient care.
- Utilize clinical assessment skills to triage incoming inquiries effectively.
- Act as a resource for staff and physicians, facilitating problem-solving and decision-making processes.
- Recommend new policies and procedures aimed at enhancing the patient experience, ensuring timely implementation.
- Assist in the initiation and coordination of patient education programs.
- Provide formal and informal teaching to staff and participate in community outreach and educational initiatives.
- Develop and implement innovative patient and family-centered care practices that promote quality, safety, and effectiveness.
Required:
- Current Registered Nurse license in the State of Illinois.
- Bachelor of Science in Nursing (BSN) with a minimum of three years of clinical experience in ambulatory care.
- Proficient in computer applications, including Microsoft Office and database management.
- Demonstrated leadership capabilities and knowledge of quality improvement methodologies.
- Ability to build clinical networks and drive systemic change.
- Skilled in managing challenging situations and balancing multiple priorities.
- Excellent verbal and written communication skills.
- Adept at data analysis for informed decision-making.
- Flexibility to adapt to changes in the work environment.
- Strong attention to detail and self-monitoring for accuracy.
- Commitment to maintaining confidentiality.
Preferred:
- Three years of clinical experience in a relevant specialty area.
- Experience in ambulatory operations.
- Background in process improvement initiatives.
