Position Summary

The Wound/Ostomy Nurse Clinician Coordinator embodies the core values of our organization, adhering to ethical standards and compliance protocols. The Coordinator is responsible for conducting thorough patient assessments and implementing appropriate nursing interventions. This position requires sound judgment, independent thought, and creativity in addressing patient care challenges, along with the coordination of educational initiatives for the patient demographic served.

Key Responsibilities
  • Deliver evidence-based care for wound and ostomy patients.
  • Perform comprehensive wound assessments and offer insights on etiology and pressure injury classification.
  • Assist the patient care team in strategies for wound prevention and treatment.
  • Conduct ostomy evaluations and provide postoperative care, education, and follow-up guidance.
  • Engage in prevalence surveys and continuous monitoring of pressure injuries.
  • Identify nursing practice trends and collaborate with leadership to enhance quality care initiatives.
  • Gather and analyze data related to pressure injuries, establishing expected outcomes and contributing to individualized care plans.
  • Coordinate care delivery and assess progress towards achieving patient outcomes.
  • Utilize the nursing process to conduct thorough patient needs assessments and propose solutions to the interprofessional team.
  • Recognize and address abnormalities identified during assessments and procedures.
  • Collaborate with clinical services and external agencies to ensure comprehensive patient care.
  • Manage incoming inquiries using clinical assessment skills and service standards.
  • Act as a resource for staff and physicians, proactively addressing patient care needs.
  • Recommend new policies and procedures aimed at enhancing the patient experience.
  • Facilitate patient education initiatives and provide training to staff.
  • Participate in community outreach and educational programs related to specialty care.
  • Innovate patient and family-centered care practices that promote quality and safety.
Qualifications

Required:

  • Active Registered Nurse license in the State of Illinois.
  • Bachelor of Science in Nursing (BSN) with a minimum of three years of clinical experience in ambulatory care.
  • Proficient in computer applications, including Microsoft Office and database management.
  • Demonstrated leadership capabilities and understanding of quality improvement methodologies.
  • Ability to build clinical networks and drive systemic changes.
  • Proficient in managing complex situations effectively.
  • Capacity to juggle multiple priorities efficiently.
  • Exceptional verbal and written communication skills.
  • Adept at data analysis for informed decision-making.
  • Flexibility to adapt to changes in the work environment.
  • Strong attention to detail and self-monitoring for accuracy.
  • Commitment to maintaining confidentiality.

Preferred:

  • Three years of clinical experience in a relevant specialty area.
  • Experience in ambulatory operations.
  • Background in process improvement initiatives.
