Position Overview

The Human Services Program Facilitator plays a crucial role in delivering essential social services to individuals and families in need. This position operates under the guidelines of state and federal regulations as well as organizational policies.

Key Responsibilities

  • Instructs and demonstrates essential life skills to clients, including budgeting, meal preparation, household management, and childcare.
  • Provides transportation for clients and their families to various appointments, including medical, dental, educational, and counseling sessions.
  • Assists clients in accessing community resources by reaching out to relevant organizations for support.
  • Helps in the development and oversight of behavior management plans, parenting classes, and maintains detailed records of actions taken on behalf of clients.
  • Engages in training clients in job readiness skills by supplying necessary materials, overseeing work tasks, and offering direct support as required.
  • Participates in mandatory training sessions and staff meetings.
  • Organizes, supervises, and documents family interactions.
  • Handles administrative tasks such as answering phones, directing calls, processing client referrals, drafting documents, filing, and managing correspondence.
  • Performs additional duties as assigned.

Qualifications

The ideal candidate will possess a high school diploma or equivalent, along with at least one year of experience in a related social services environment. Additional qualifications may be considered upon review and approval.

Skills and Competencies

  • Familiarity with agency and community resource networks.
  • Proficiency in relevant computer applications.
  • Ability to effectively teach and model practical living skills.
  • Competence in observing and assessing client interactions and case progress.
  • Strong interviewing skills to gather and analyze client information.
  • Ability to prepare and deliver both oral and written reports.
