About the Role:

As a Discharge Planning Assistant, you will play a vital role in ensuring a smooth transition for our patients from our center to their next level of care.

Key Responsibilities:

  • Provide comprehensive medically related social services to patients, including MDS, Care Plans, Assessments, and timely documentation.
  • Operate within the framework of our company's standards for best practice in accordance with all state and federal regulations.
  • Collaborate with our interdisciplinary team to develop and implement effective discharge plans.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions.

Requirements:

  • A 4-year degree in Social Work, Psychology, Sociology, Counseling, Criminal Justice, or other human services related degree.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
