PATH is seeking compassionate individuals eager to make a meaningful difference in the lives of others to join our Supportive Housing Services (SHS) team.
ABOUT OUR SUPPORTIVE HOUSING SERVICES
Our SHS programs deliver Intensive Case Management Services (ICMS) within a Permanent Supportive Housing (PSH) framework for individuals who have faced homelessness and have complex health and mental health needs. Our services are designed to be accessible, ensuring clients receive the necessary resources for long-term housing stability and improved quality of life.
ROLE OF A CASE MANAGER
As a Case Manager, you will collaborate with a dedicated team of community service providers and PATH staff to support PSH tenants in achieving their personal goals. Your responsibilities will include:
- Conducting regular meetings with tenants to provide strengths-based case management and service coordination, including support for independent living skills and access to community resources.
- Working with tenants to develop personalized plans that address both immediate and long-term objectives, along with follow-up to ensure progress.
- Participating in team consultations to perform crisis assessments and provide intervention services aimed at enhancing tenants' problem-solving abilities and coping strategies.
- Advocating for tenants to secure and maintain essential resources such as housing, healthcare, and transportation.
QUALIFICATIONS
We are looking for candidates who:
- Are committed to providing community and in-home services regularly.
- Exhibit a high level of empathy and understanding for individuals with complex needs.
- Can work effectively both independently and as part of a team.
- Demonstrate sound judgment, motivation, and a proactive approach.
- Possess strong communication skills, both written and verbal.
- Can prioritize tasks and manage multiple demands in a fast-paced environment.
PREFERRED EXPERIENCE
Ideal candidates will have:
- Two years of experience in healthcare, case management, homeless services, or a related field.
MINIMUM REQUIREMENTS
All candidates must meet the following:
- All levels of education and experience are welcome.
MANDATORY CRITERIA
Regardless of qualifications, candidates must:
- Provide employment eligibility verification.
- Obtain CPR/First Aid certification.
- Complete a Tuberculosis Test, Background Screening, and Drug Test as a condition of hire.
DRIVING REQUIREMENTS
Driving is a crucial aspect of this role. Candidates must:
- Have reliable transportation.
- Possess a valid driver's license.
- Provide proof of insurance and ownership for personal vehicles used for work purposes.
- Meet the criteria for PATH's insurance coverage.
