Interim Housing Case Manager

LA Family Housing is seeking a highly motivated and compassionate Interim Housing Case Manager to join our team. As a key member of our Programs - Interim Housing team, you will play a vital role in assisting participants with assessing and connecting to community resources.
Job Summary:The Interim Housing Case Manager is responsible for providing support and advocacy through crisis prevention and intervention, conflict resolution, and intensive case management. You will work closely with participants to develop individualized plans to address their goals, such as income, medical, mental health, substance use, education, childcare, and social support.
Key Responsibilities:- Assess participant needs and develop individualized plans to address goals
- Provide ongoing strength-based case management services and coordination with participants
- Offer referrals, linkages, information, and support to connect participants with needed resources
- Assist participants in obtaining necessary documentation for housing connections
- Collaborate with internal and external providers to ensure effective care coordination
- Maintain accurate records using written forms and HMIS/CHAMP
- Conduct crisis and risk assessments, and develop safety plans as needed in consultation with supervisors
- Provide crisis intervention and conflict management support when necessary
- Follow mandated reporting guidelines, communicating with protective and emergency services as required, and completing follow-up documentation, including incident reports
- Attend regular staff, agency, and community meetings as directed by supervisors
- Stay informed about best practices and interventions related to Domestic Violence, Harm Reduction, Trauma-Informed Care, and the Housing First Model
- Demonstrate flexibility in schedule and site locations, as needed for multiple sites
- At least one (1) year of relevant work experience
- Experience in dealing with substance abuse, mental health, and crisis intervention desired
