Case Manager

PATH is seeking a dedicated and compassionate Case Manager to join our Interim Housing team in Signal Hill, CA. As a Case Manager, you will play a vital role in supporting individuals experiencing homelessness by providing individualized case management services, connecting them to essential resources, and advocating for their needs.
Job Summary:The Case Manager will work closely with clients to assess their needs, develop individualized service plans, and provide referrals to resources that promote housing stability, income growth, and overall well-being. This is an excellent opportunity for a motivated and empathetic professional to make a meaningful difference in the lives of our clients.
Responsibilities:- Conduct intake and individualized needs assessments for clients, collaborating to develop and implement effective service plans.
- Provide referrals, linkages, information, and support to resources that help clients achieve their goals.
- Evaluate strengths and challenges to address short-term and long-term goals, conducting regular reviews with clients.
- Maintain accurate and up-to-date client files, adhering to documentation standards and confidentiality guidelines.
- Develop therapeutic and professional relationships with clients and stakeholders to enhance engagement and retention.
- Two years of experience in healthcare, case management, homeless services, or a related field.
- Excellent analytical, communication, and problem-solving skills.
- Ability to work effectively in a team environment and maintain confidentiality.
- Valid driver's license, reliable transportation, and proof of insurance.
