Job Summary

The Shelter Case Manager is a vital member of our team at The Whole Child - Mental Health & Housing Services, dedicated to supporting families in breaking the cycle of homelessness and working towards permanent housing. This role requires a compassionate and skilled professional to provide day-to-day support to families residing at our shelter facility, while also linking them to essential services.

Key Responsibilities
  • Conduct thorough intakes and exits with families entering our shelter, ensuring a smooth transition and addressing their unique needs.
  • Implement Trauma-Informed, Housing First, Harm Reduction, and Low Barrier approaches to provide comprehensive support to our clients.
  • Maintain accurate and timely progress notes in our electronic record system, adhering to a 72-hour timeframe.
  • Offer 'light touch' case management services, assisting families in preparing for permanent housing transitions and developing personalized housing plans.
  • Collaborate with clients to identify and address short-term and long-term housing goals, ensuring a stable and secure future.
  • Support the enrollment and exit process for participants in our Interim Housing Program, utilizing HMIS effectively.
  • Assist with meal service for shelter families, working closely with the Shelter Manager.
  • Develop corrective action plans with families, addressing challenges and promoting positive change.
  • Participate in monthly fumigation activities and conduct safety walk-throughs of the facility, ensuring a safe environment for all.
  • Identify and connect families with internal and external resources to address multiple stressors related to homelessness, domestic violence, substance abuse, and more.
  • Adopt a 'whatever-it-takes' approach to assist clients in retaining housing and independent living, demonstrating a commitment to their success.
  • Conduct quarterly client file reviews, ensuring quality assurance and adherence to safety and housing practice protocols.
  • Aid shelter staff in addressing crisis situations, upholding the Grievance Process, and maintaining confidentiality in all matters.
  • Exercise sound judgment in the performance of all duties and responsibilities, upholding professional conduct and maintaining good boundaries.
Requirements
  • Bachelor's degree in social work, psychology, or a related mental health discipline.
  • Minimum of 4 years of experience in providing Housing Navigation or rehabilitative services in a mental health setting.
  • Valid California driver's license and valid auto insurance.
