Job Description

The Case Manager plays a vital role in ensuring the delivery of quality care to residents living in Project Based housing. As a team member, they work closely with social service professionals and paraprofessionals to provide comprehensive support services.

Key Responsibilities:
  • Provide high-quality on-site case management services to residents, serving as the central point of contact for clients navigating the transition from homelessness to tenant living.
  • Evaluate individual/families for permanent housing in conjunction with the housing program manager and other case management staff.
  • Assist individuals/families to establish eligibility for subsidized housing, collecting required documents and completing applications.
  • Develop professional relationships with clients, maintaining a caseload of project-based participants, and conducting regular face-to-face case management sessions.
  • Assist with the preparation of forms and documentation required for initial housing voucher approval with the Housing Authority.
  • Conduct regular on-site office hours at assigned properties, conducting initial home visits upon new tenant lease-up and ongoing quarterly home visits as needed.
  • Conduct ongoing monthly case management sessions to monitor clients' progress, providing community referrals, and advocating on behalf of clients when necessary.
  • Provide education on topics related to successful housing retention, decreasing social isolation, and preventing relapse risks, including setting up and transporting clients on field trips.
  • Support clients in developing independent living skills, including money management, budgeting, housekeeping, nutrition, and social/recreational activities.
  • Develop and implement individualized service plans, emphasizing coordinated strategies for addressing short and long-term housing needs, financial stability, and other risk factors.
  • Assist clients with maintaining medication and treatment regimens, accompanying/transporting clients to appointments in the community.
  • Utilize a harm reduction, client-focused, and strengths-based model, ensuring case notes are entered into the Homeless Management Information System within 48 hours of completing direct case management services.
Requirements:
  • Bachelor's degree from an accredited college or university in social work, human services, or a related field.
  • One to two years of experience providing services to highly vulnerable populations.
  • Strong knowledge of housing market, housing programs, and resources, with demonstrated knowledge and experience with Harm Reduction, Motivational Interviewing, Critical Time Intervention, and Housing First.
  • Excellent organizational skills, with the ability to work in a fast-paced environment.
  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and ability to use widely supported internet browsers.
  • Valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.
