The Youth Care Services Coordinator (YCSC) role encompasses not only coordination but also assessing and addressing the fundamental needs of the youth through consistent engagement with their families, aiming to make a meaningful difference in their lives using the Wraparound Service Model. We assist families in recognizing minor advancements that culminate in significant transformations. For each family you support, you will serve as the primary liaison for the youth's service providers, while also crafting a personalized roadmap that ensures cohesive treatment among the team. As the main contact, you will collaborate with multidisciplinary teams, including primary care providers, educational institutions, child welfare services, outpatient therapists, and other mental health or medical professionals.

Starting Salary:

Key Responsibilities:

  • Collect information to complete comprehensive home-based assessments focused on strengths.
  • Assist in identifying and nurturing natural supports within the family's network.
  • Organize regular Care Planning Team meetings.
  • Create a youth and family-centered Individual Care Plan (ICP) that aligns with the goals articulated by the family, in collaboration with care providers, natural supports, and other team members.
  • Maintain consistent communication with the youth, family, and other providers through phone calls, community meetings, or home visits to ensure the ICP is effectively implemented.
  • Provide education, advocacy, and support to youth and families navigating various child and family-serving systems.
  • Identify and connect youth and families to additional community resources and services through referrals and care coordination.
  • Develop a risk management and safety plan, offering crisis support as needed, and coordinate with the Mobile Crisis Intervention team.

Qualifications:

  • Education:
    • Bachelor's degree in a relevant mental health field is required (including but not limited to counseling, family therapy, social work, psychology, etc.).
    • Master's degree is preferred.
  • Skills:
    • Must possess or be able to obtain Child and Adolescent Needs and Strengths (CANS) certification within the first month of employment.
    • Valid Driver's License and reliable transportation are required.
    • The ideal candidate should excel in fast-paced environments, demonstrate strong attention to detail, and possess excellent organizational and multitasking abilities.
    • Bilingual candidates are strongly encouraged to apply; we offer a $5000 language differential pay. Please indicate language skills on your resume.
  • Experience:
    • Familiarity with programs serving children and youth is preferred.
