Job Overview

North Suffolk Community Services Inc is seeking a Senior Care Coordinator to enhance our service delivery. This role is pivotal in ensuring that children with behavioral and emotional challenges, along with their families, achieve independence within their communities. The Senior Care Coordinator's responsibilities extend beyond mere coordination; it involves evaluating and identifying the core needs of youth through consistent engagement with families, thereby making a significant positive impact in their lives using the Wraparound Service Model. Additionally, this position includes providing oversight and leadership to fellow Care Coordinators.


Key Responsibilities:

  • Offer leadership and clinical as well as administrative guidance to assigned Care Coordinators.
  • Assist the Program Manager in onboarding and training new Care Coordinators.
  • Collect information for comprehensive, strengths-based home assessments and aid in identifying and developing natural supports within the family’s network.
  • Organize, facilitate, and document regular Care Planning Team meetings.
  • Create a youth and family-centered Individual Care Plan (ICP) that aligns with the goals identified through the family's voice and choice, in collaboration with care providers and other team members.
  • Maintain ongoing communication with the youth, family, and other providers through phone calls, community meetings, or home visits to ensure the effective implementation of the ICP.
  • Provide education, advocacy, and support to youth and families navigating various child and family-serving systems.
  • Identify and connect youth and families to additional community supports and services through referral and care coordination.
  • Develop a risk management and safety plan, providing crisis support as needed, in coordination with the Mobile Crisis Intervention team.
  • Assist the Program Manager with processing incoming referrals and managing the waitlist.
  • Support the Program Manager in implementing Utilization Review.
  • Provide on-call support under the guidance of the Program Manager.

Qualifications:

  • Education:
    • Master's degree in a relevant field is required.
    • Independent licensure is preferred.
  • Skills and Experience:
    • Possession of a valid Driver's License and access to a reliable vehicle.
    • A minimum of three years of experience working with families of children and adolescents with Serious Emotional Disturbances (SED).
    • At least one year of supervisory experience.
    • The ideal candidate should excel in fast-paced environments, demonstrate strong attention to detail, and possess excellent organizational and multitasking abilities.
    • Bilingual candidates (Spanish or Portuguese) are strongly encouraged to apply, with a $5000 language differential pay available. Please indicate language skills on your resume.
