Key Responsibilities- Gather and analyze information to complete comprehensive home-based assessments, identifying strengths and needs of clients and their families.
- Develop and implement individualized care plans that support the achievement of client goals, in collaboration with multidisciplinary teams and natural supports.
- Facilitate regular care planning meetings to ensure effective communication and coordination among team members.
- Provide education, advocacy, and support to clients and families navigating child-serving systems, connecting them to additional resources and services as needed.
- Develop and maintain risk management and safety plans, providing crisis support and coordination with mobile crisis intervention teams as required.
- Education: Bachelor's degree in a relevant mental health field (e.g., counseling, family therapy, social work, psychology) is required; Master's degree is preferred.
- Certifications: Must have or be able to acquire Child and Adolescent Needs and Strengths (CANS) certification within the first month of employment.
- Skills: Strong attention to detail, organization, and multitasking skills are essential. Ideal candidate thrives in fast-paced work environments and demonstrates strong communication and interpersonal skills.
- Experience: Familiarity with child and youth-serving programs is preferred.
