Position Overview

North Suffolk Community Services Inc. is seeking a Senior Care Coordinator to enhance our service delivery. This role is pivotal in empowering children with behavioral and emotional challenges, alongside their families, to achieve independence within their communities.

The Senior Care Coordinator goes beyond mere coordination; it involves assessing and addressing the core needs of youth through consistent engagement with families. By utilizing the Wraparound Service Model, this position aims to create a significant positive impact in the lives of those we serve while providing guidance and leadership to fellow Care Coordinators.

Key Responsibilities:
  • Offer leadership and clinical supervision to assigned Care Coordinators.
  • Assist the Program Manager in onboarding and training new team members.
  • Conduct comprehensive home-based assessments focusing on strengths and identify natural supports within the family network.
  • Organize and document regular Care Planning Team meetings.
  • Develop individualized care plans that reflect the goals and preferences of families, in collaboration with care providers and other team members.
  • Maintain ongoing communication with youth, families, and service providers through various means, including phone calls and home visits, to ensure effective implementation of care plans.
  • Provide education and advocacy for families navigating child and family-serving systems.
  • Connect families to additional community resources and services through referrals and coordinated care.
  • Develop risk management and safety plans, offering crisis support as needed, in collaboration with the Mobile Crisis Intervention team.
  • Assist the Program Manager with processing incoming referrals and managing waitlists.
  • Support the implementation of Utilization Review processes.
  • Provide on-call support as directed by the Program Manager.
Qualifications:
  • Education:
    • Master's degree in a relevant field is required.
    • Independent licensure is preferred.
  • Skills and Experience:
    • Valid Driver's License and access to a reliable vehicle.
    • A minimum of three years of experience working with families of children with Serious Emotional Disturbances (SED).
    • At least one year of supervisory experience is required.
    • The ideal candidate will excel in fast-paced environments, demonstrating strong attention to detail and excellent organizational skills.
    • Bilingual candidates (Spanish or Portuguese) are strongly encouraged to apply, with a $5000 language differential available.
