Position Overview

North Suffolk Community Services Inc. is seeking a Senior Care Coordinator to enhance our dedicated team. Our mission is to empower children with behavioral and emotional challenges, along with their families, to achieve independence within their communities. This role encompasses not only coordination but also the assessment and identification of the core needs of youth through consistent engagement with families, aiming to create a significant positive impact in their lives utilizing the Wraparound Service Model.

Key Responsibilities:

  • Provide leadership and clinical supervision to assigned Care Coordinators.
  • Assist the Program Manager in onboarding and training new staff members.
  • Conduct comprehensive home-based assessments to identify strengths and develop natural supports within the family network.
  • Organize and document regular Care Planning Team meetings.
  • Create individualized care plans that reflect the goals identified by the family, in collaboration with care providers and other team members.
  • Maintain ongoing communication with youth, families, and service providers through various methods including phone calls and home visits.
  • Offer education, advocacy, and support to families navigating child and family-serving systems.
  • Connect families to additional community resources and services through effective referral and care coordination.
  • Develop risk management and safety plans, providing crisis support as necessary.
  • Assist with the processing of incoming referrals and management of waiting lists.
  • Support the implementation of Utilization Review processes.
  • Provide on-call support as directed by the Program Manager.

Qualifications:

  • Education: Master’s degree in a relevant field is required; independent licensure is preferred.
  • Experience: Minimum of three years working with families of children and adolescents with Serious Emotional Disturbances (SED); at least one year in a supervisory role.
  • Skills: Strong organizational and multitasking abilities, attention to detail, and the capacity to thrive in a fast-paced environment. Bilingual candidates (Spanish or Portuguese) are strongly encouraged to apply, with a $5000 language differential available.
