Caregiver Support Specialist

NORTH SUFFOLK COMMUNITY SERVICES INC is seeking dedicated Family Partners to enhance our Community Service Agency. In this role, you will cultivate a supportive, one-on-one, strength-based relationship with a parent or caregiver of a youth facing serious emotional challenges. Your primary focus will be to address the emotional and behavioral needs of the youth while empowering their caregivers.
Ideal Candidate: This position is tailored for individuals with personal experience as a parent or caregiver of a youth with special needs, particularly in the realm of mental health.
Work Environment: This role offers a hybrid work model, allowing for community visits alongside remote work based on family preferences.
Key Responsibilities:
- Engage parents and caregivers through activities in both home and community settings.
- Support caregivers in achieving goals outlined in the youth's treatment plan for Outpatient or In-Home Therapy.
- Coordinate Intensive Care Coordination Services with the agency's Care Coordinator and external Hub providers.
- Educate, support, and empower caregivers to meet the needs of the youth through various methods such as coaching and guidance.
- Deliver services in the caregiver's home and local community.
- Attend and provide support during Individual Care Plan meetings alongside caregivers and Care Coordinators.
- Maintain consistent communication with caregivers through face-to-face meetings and phone calls.
- Offer family education, advocacy, and support to both youth and their families.
- Regularly review and update the Individual Care Plan to reflect the evolving needs of the youth and family.
- Adhere to all policies and procedures regarding client rights and confidentiality.
- Ensure timely and accurate documentation of service delivery.
Qualifications:
- Education:
- Bachelor's degree in a Human Services field with one year of relevant experience OR
- Associate's degree in a human service field with one year of related experience OR
- High School diploma/GED with a minimum of two years of relevant experience.
- Experience:
- Required lived experience as a parent or caregiver of a youth with special needs, preferably in mental health.
- Experience working with youth exhibiting Serious Emotional Disturbance.
- Familiarity with navigating child and family service systems.
- Knowledge of child and youth service programs.
- Skills:
- Bilingual in Spanish is preferred (please indicate language proficiency on application).
- A valid driver's license and willingness to travel for offsite visits are required.
