Position Overview

NORTH SUFFOLK COMMUNITY SERVICES INC is seeking dedicated Family Support Specialists to become integral members of our Community Service Agency. In this role, you will establish a structured, strength-based relationship with parents or caregivers of youth facing serious emotional challenges. Your primary focus will be to address the emotional and behavioral needs of these youths while empowering their families.

Ideal Candidate: This position is tailored for individuals who have firsthand experience as a parent or direct caregiver of a youth with special needs, particularly those with mental health challenges.

Work Environment: This role offers a hybrid work model, allowing for community visits alongside remote work, based on family preferences.

Key Responsibilities:

  • Engage with parents/caregivers through activities in both home and community settings.
  • Support parents/caregivers in achieving goals outlined in the youth's treatment plan, whether for Outpatient or In-Home Therapy.
  • Coordinate Intensive Care Coordination Services in collaboration with Care Coordinators and external Hub providers.
  • Assist parents/caregivers in meeting their youth's needs through education, support, coaching, and empowerment.
  • Deliver services directly in the home and community environments of the families.
  • Participate in and support parents/caregivers during Individual Care Plan meetings.
  • Maintain consistent communication with parents/caregivers through face-to-face interactions and phone calls.
  • Provide educational resources, advocacy, and support to families.
  • Regularly monitor and update the Individual Care Plan to reflect the evolving needs of the youth and family.
  • Ensure adherence to policies regarding client rights and confidentiality.
  • Complete necessary documentation of service delivery promptly.

Qualifications:

  • Education:
    • Bachelor's degree in a Human Services field with one year of relevant experience OR
    • Associate's degree in a human services field with one year of related experience OR
    • High School diploma/GED with a minimum of two years of relevant experience.
  • Experience:
    • Required lived experience as a parent or direct caregiver of a youth with special needs, particularly mental health needs.
    • Experience working with parents/caregivers and youth with Serious Emotional Disturbance.
    • Familiarity with navigating child and family service systems.
    • Knowledge of child and youth service programs.
  • Skills:
    • Bilingual in Spanish is preferred (please indicate language proficiency in your application).
    • A valid driver's license and willingness to travel as needed.
