Job Description

Elite Home Health Care is a licensed home care agency that provides professional homecare services in the New York Metropolitan area, Central, and Upstate NY Regions. At Elite, we only hire the most qualified nurses, home health aides and caregivers who are hand-picked based on their skills and capabilities. They are all licensed, insured and cleared with extensive background screening. But most importantly, our caregivers are known for their warmth, attentiveness and reliability. Job Summary

Elite Home Health Care is seeking a highly motivated and results-driven individual to join our team as a Community Outreach Representative. This role will be responsible for establishing and maintaining relationships within the community to generate quality leads for our home care services.

Key Responsibilities:
  • Submit daily reports to management to ensure seamless communication and goal achievement.
  • Build relationships with hospitals, nursing homes, community organizations, and physician practices to promote our agency and generate referrals.
  • Develop and manage new referral sources, including nursing homes, hospitals, and community referrals.
  • Conduct presentations to support groups, adult day cares, and community events to promote our services and complete the enrollment process.
  • Follow up on leads and referrals from family members, caregivers, and other sources to ensure timely and effective communication.
Requirements:
  • Minimum of 1 year of sales experience with a proven track record of success.
  • Ability to build relationships and hold events within the community.
  • Excellent communication and interpersonal skills.
  • Self-motivated and results-driven individual with a strong work ethic.
  • Must have a car and be able to travel to meetings daily.
  • **Preferred: Bilingual in Spanish and English**
Work Environment:
  • On-the-road sales and marketing role.
  • 8-hour shift with flexibility to accommodate client needs.

Pay Range: The hiring range for this position is $55,000.00 annually, with final compensation determined by factors such as years of relevant work experience, skills, certifications, and location.
