Job Overview

POSITION SUMMARY:

The Community Support Specialist plays a vital role within a multidisciplinary care coordination team, providing outreach, assistance, and care management for individuals eligible for the Forensic ACT Team. This includes support for those facing chronic medical conditions alongside mental health and/or substance use challenges. The Specialist acts as a mentor and health coach, fostering recovery and wellness.


QUALIFICATIONS:

· High School Diploma or GED required.

· Relevant personal experience is essential: lived experience with disabilities and/or emotional challenges, and a commitment to using this experience to support others in their recovery journey.


REQUIRED SKILLS:

· Valid New York State Driver's License.

· Clean driving record.

· Proof of vehicle insurance.


WORK ENVIRONMENT:

This role requires frequent sitting, talking, and listening. Occasional walking and the use of hands for various tasks are necessary. The ability to lift and/or move items up to 10 pounds may be required. Specific vision capabilities include close vision and the ability to adjust focus.


KEY RESPONSIBILITIES:

  • Engage clients to promote their participation in services offered by the Forensic ACT Team.
  • Deliver health coaching and recovery support, focusing on self-advocacy, wellness management, medication adherence, and daily living skills.
  • Assist in coordinating transportation for clients to appointments, including travel training when necessary.
  • Accompany clients to medical and other appointments to ensure their engagement and follow-through.
  • Provide essential services to clients, including assistance with benefits and enrollments.
  • Maintain organized and accurate documentation, records, and reports in accordance with policies.
  • Collaborate with other service providers to ensure coordinated and goal-oriented care.
  • Participate in team meetings to discuss client progress and contribute to care planning.
  • Serve as a resource for team members regarding psychosocial and substance use issues.
  • Conduct outreach and service planning both in-person and via phone.
  • Link clients to community resources, including medical, behavioral, and residential services.
  • Monitor service delivery to ensure continuity and advocate for clients as needed.
  • Provide crisis intervention and follow-up support.
  • Facilitate the sharing of information with clients to promote informed decision-making.
  • Complete quality improvement training during onboarding and participate in ongoing training as required.
  • Encourage and implement quality improvement initiatives as part of professional development.
  • Foster an inclusive workplace culture that values diverse perspectives and experiences.
  • Adhere to safety protocols to maintain a secure working environment.
  • Perform additional duties as assigned.
