Job Summary

CN Guidance and Counseling Services is seeking a highly motivated and compassionate Peer Specialist - Recovery Coach to join our team. As a Peer Specialist, you will play a vital role in supporting individuals with mental health and substance use conditions in their recovery journey.

Key Responsibilities
  • Provide outreach and engagement services to clients, facilitating their access to healthcare and social services.
  • Offer health coaching and recovery support, empowering clients to take control of their well-being and make informed decisions about their care.
  • Assist with transportation and accompaniment to medical and other appointments, ensuring clients receive the support they need to navigate the healthcare system.
  • Maintain accurate and timely records, statistics, and reports, adhering to agency policies and procedures.
  • Collaborate with interdisciplinary care teams to ensure coordinated and goal-directed care, including care transitions and crisis intervention.
  • Act as a resource and consultant to team members on psychosocial and substance abuse issues, providing expertise and support to enhance client outcomes.
  • Provide telephonic and in-person outreach, engagement, and service planning in the field, linking clients to community resources and services.
  • Advocate on behalf of clients to ensure access to necessary services and support, promoting a culture of inclusion and respect.
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing strategies to enhance client care and agency operations.
Requirements
  • High School Diploma or equivalent required; relevant life experience and personal experience living with a disability or social/emotional challenge preferred.
  • Current New York State Driver's License and clean DMV record required.
  • Proof of motor vehicle insurance and ability to maintain a safe driving record.
  • Ability to lift and move up to 10 pounds, with frequent sitting, talking, and hearing.
  • Specific vision abilities required, including close vision and ability to adjust focus.
