Peer Specialist

CN Guidance and Counseling Services is seeking a highly motivated and compassionate Peer Specialist to join our interdisciplinary care coordinator team. As a Peer Specialist, you will play a vital role in supporting individuals with mental health and substance use conditions, providing outreach, health coaching, and recovery support.
Key Responsibilities- Provide outreach and engagement services to clients, facilitating their access to mental health and substance use treatment.
- Offer health coaching and recovery support, empowering clients to manage their mental health and wellness.
- Coordinate care planning with other providers, ensuring goal-directed and collaborative care.
- Act as a resource and consultant to team members on psychosocial and substance abuse issues.
- Provide crisis intervention and follow-up services as needed.
- Monitor and report on service delivery, ensuring coordination and continuity of care.
- High School Diploma or equivalent required.
- Relevant life experience, including personal experience living with a disability or social/emotional challenge.
- Current New York State Driver's License and clean DMV record required.
- Ability to work in a fast-paced environment and adapt to changing circumstances.
