Community Support Specialist

POSITION SUMMARY:
The Community Support Specialist is an integral member of a multidisciplinary care coordination team dedicated to outreach, support, and the management of care for individuals eligible for and/or receiving services from the Forensic ACT Team. This role focuses on assisting individuals who are chronically medically ill and may also face co-occurring mental health and/or substance use challenges. The Specialist serves as a mentor and health coach, promoting recovery and wellness.
QUALIFICATIONS:
· High School Diploma or GED required.
· Relevant personal experience is essential: individuals who have lived with a disability and/or emotional challenges are encouraged to apply, as this experience is vital in supporting others in their recovery journey.
REQUIRED SKILLS:
· Valid New York State Driver's License.
· Clean driving record.
· Proof of vehicle insurance.
PHYSICAL REQUIREMENTS:
In the performance of this role, the employee is frequently required to sit, communicate, and listen. Occasional walking, using hands and fingers to manipulate objects, and reaching with arms are also necessary. The ability to lift and/or move up to 10 pounds may be required. Specific vision abilities include close vision and the ability to adjust focus.
KEY RESPONSIBILITIES:
- Engage with clients to encourage their participation in services offered by the Forensic ACT Team.
- Provide health coaching and recovery support, focusing on self-advocacy, wellness management, medication adherence, smoking cessation, and daily living skills.
- Assist in coordinating transportation for clients to appointments, including travel training as necessary.
- Accompany clients to medical and other appointments to enhance their engagement and follow-through.
- Deliver concrete services to clients, such as benefits enrollment, according to program requirements.
- Maintain organized and accurate documentation, records, and reports in compliance with policies and procedures.
- Collaborate with other service providers to ensure coordinated, goal-oriented care, including transitions of care.
- Participate actively in team meetings, providing insights on psychosocial conditions and client progress.
- Serve as a resource for team members regarding psychosocial and substance use issues.
- Conduct outreach and service planning both telephonically and in person.
- Facilitate connections to community resources, including medical, behavioral, and residential services.
- Monitor service delivery to ensure continuity and advocate for clients with service providers.
- Provide crisis intervention and follow-up support.
- Share relevant data with clients to promote informed decision-making and progress tracking.
- Complete Quality Improvement (QI) training during onboarding and participate in ongoing QI education.
- Identify and implement quality improvement initiatives as part of professional development.
- Foster an inclusive workplace culture that values diverse perspectives and experiences.
- Adhere to safety protocols to maintain a secure working environment.
- Perform additional duties as assigned.
