POSITION SUMMARY:
The Community Support Specialist plays a crucial role within a multidisciplinary care coordination team, providing outreach, assistance, and care management for individuals eligible for the Forensic ACT Team. This includes supporting those with chronic medical conditions alongside mental health and/or substance use challenges. The Specialist acts as a mentor and health coach, guiding clients through their recovery journey.
EDUCATIONAL BACKGROUND AND EXPERIENCE:
· High School Diploma or GED required.
· Relevant personal experience is essential: lived experience with a disability and/or emotional challenges, with a commitment to utilizing this background to inspire and support others in their recovery process.
ESSENTIAL QUALIFICATIONS:
· Valid New York State Driver's License.
· Clean driving record.
· Proof of automobile insurance.
PHYSICAL REQUIREMENTS:
In performing the responsibilities of this position, the employee is frequently required to sit, communicate, and listen. Occasionally, the employee must walk, use hands and fingers to handle or operate objects, tools, or controls, and reach with hands and arms. The employee may occasionally lift and/or move items weighing up to 10 pounds. Specific vision abilities required include close vision and the ability to adjust focus.
KEY RESPONSIBILITIES:
- Engage clients through outreach efforts to encourage their participation in services offered by the Forensic ACT Team.
- Provide health coaching and recovery support, focusing on self-advocacy, wellness management, medication adherence, smoking cessation, entitlements, and daily living skills.
- Assist in coordinating transportation for clients to appointments, including offering travel training as necessary.
- Accompany clients to medical and other appointments to enhance their engagement and follow-through.
- Deliver essential services to clients, such as benefits enrollment and assistance tailored to program requirements.
- Maintain organized, timely, and accurate documentation, records, statistics, and reports in accordance with established policies.
- Collaborate with other service providers to ensure coordinated, goal-oriented care, including transitions of care.
- Participate actively in team meetings, contributing insights on psychosocial conditions and client progress to refine care plans and objectives.
- Serve as a resource for team members regarding psychosocial and substance use issues.
- Conduct outreach and service planning both telephonically and in person.
- Facilitate connections to community resources, including medical, behavioral, residential, and entitlement services as per the interdisciplinary care plan.
- Monitor service delivery to clients to ensure continuity and advocate with service providers as needed.
- Provide crisis intervention and follow-up support.
- Encourage shared decision-making by keeping clients informed of their progress.
- Complete Quality Improvement (QI) training during onboarding and participate in ongoing QI training as required.
- Identify and implement quality improvement initiatives as part of professional development.
- Promote a culture of inclusion that values individual strengths and experiences, fostering better decision-making and innovation.
- Adhere to safety protocols to maintain a secure working environment for all employees.
- Perform additional duties as assigned.
