Position Summary:
LifeMoves' Julian Street Inn is currently in search of a skilled and motivated Bilingual (Spanish Speaking) Case Manager. This facility provides emergency and transitional housing for adults striving to overcome homelessness and mental health challenges. Under the guidance of the Program Director, Case Managers serve as essential supports and advocates for clients, aiming to assist them in attaining stable housing and self-sufficiency. Responsibilities include developing and executing individualized case plans for a designated caseload, as well as aiding the Program Director in maintaining facilities and enhancing program services.

Key Responsibilities:
  • Formulate, implement, and adjust individual case plans while consistently monitoring client progress.
  • Provide ongoing support and case management in alignment with established treatment plans.
  • Act as a professional liaison between LifeMoves and community partners.
  • Communicate effectively with probation officers, social workers, and family members as necessary.
  • Engage actively as a team member in supporting clients and the facility.
  • Stay informed about community resources, referrals, and service linkages.
  • Deliver crisis management for clients and facilitate appropriate interventions.
  • Lead at least one clinical or life skills group in addition to morning sessions.
  • Maintain client and program records in compliance with applicable standards and regulations.
  • Uphold a high level of ethical conduct regarding confidentiality and professional relationships.
  • Participate in continuing education to remain knowledgeable in relevant areas.
  • Attend weekly case consultation meetings and monthly training sessions; regularly meet with supervisors to exchange information and receive guidance.
  • Perform other duties as assigned by supervisors.
Qualifications:
  • Bachelor's degree in social work, psychology, or a related field, with a minimum of two years of case management experience.
  • Preferred minimum of two years of experience in chemical dependency or related treatments.
  • Strong communication and interpersonal skills; proven ability to collaborate with community partners.
  • Understanding of challenges faced by homeless individuals and marginalized populations in crisis.
  • Availability to work occasional evenings and weekends.
  • Experience working with clients who are mentally ill or dually diagnosed is highly desirable.
  • Commitment to diversity, equity, and inclusion, along with ongoing professional development in these areas.
  • Bilingual Spanish-speaking required.
