Program Overview:
The HomeFirst Citywide Outreach and SOAR Project aims to connect individuals experiencing homelessness in urban areas to essential services such as shelter, case management, mental health support, and housing solutions. This initiative primarily targets vulnerable, chronically homeless individuals residing on the streets or in encampments across the region. Staff members are dedicated to establishing a consistent presence in outdoor locations to foster trust and rapport with unsheltered individuals. The ultimate goal is to link participants with services that facilitate their transition to permanent housing. The outreach team comprises Outreach Specialists, Case Managers, Drug and Alcohol Counselors, Clinicians, and supervisory roles.
Key Responsibilities:
Direct Services (70%):
- Deliver street and encampment-based case management to individuals in need at designated locations.
- Facilitate referrals and connections to internal Clinicians and Alcohol and Drug Counselors.
- Coordinate services with community resources, including healthcare, benefits applications, employment counseling, and family reunification.
- Collaborate with the Program Manager while maintaining accurate service records.
- Engage with community partners to enhance support services for participants.
- Utilize supervision effectively, ensuring open communication and updates on client intake activities.
- Implement agency and program policies and procedures diligently.
- Report any suspected cases of abuse in accordance with protocols.
- Maintain professional relationships and conduct at all times.
- Document all case management activities accurately as required by grant contracts.
- Conduct client intake assessments as necessary.
- Work with participants to create and execute individualized case management plans, providing support to achieve housing and personal objectives.
- Meet regularly with participants based on their needs.
- Offer support services employing a Critical Time Intervention (CTI) model.
- Arrange transportation for participants to shelters and critical appointments.
- Provide weekend and after-hours support as needed.
- Assist with additional duties as assigned.
- Record all case management activities in HMIS within two business days and maintain comprehensive case file documentation.
- Engage in ongoing program analysis to align with industry best practices.
- Continuously seek to enhance the quality and effectiveness of program services.
- Conduct regular reassessments of client needs and document findings in HMIS.
- Attend all relevant meetings and training sessions.
- Assist with office-related tasks as necessary.
- Maintain open lines of communication with supervisors and provide regular updates on activities.
- Adhere to agency and program policies and procedures.
- Report any suspected cases of abuse appropriately.
- Perform other duties as assigned.
- Bachelor's degree with relevant experience in nonprofit organizations (or equivalent experience in case management).
- Understanding and sensitivity to the needs of the homeless population.
- Experience in building relationships with community partners and external agencies.
- Ability to manage multiple projects and adapt to changing priorities.
- Proven organizational skills and ability to meet deadlines.
- Strong accountability and professionalism.
- Effective communication skills, both written and verbal.

Each year, we assist over 6,000 adults, veterans, families, and young adults across multiple locations in Santa Clara County and southern San Mateo County.
