Program Overview:

The HomeFirst Citywide Outreach and SOAR initiative aims to connect individuals experiencing homelessness in street and encampment settings with essential services, including shelter, case management, mental health support, and housing solutions. This program focuses on vulnerable, chronically homeless individuals residing in various locations throughout San Jose. Our dedicated staff members will establish a consistent presence in these areas to foster trust and rapport with unsheltered residents. The ultimate goal is to facilitate access to services that lead to permanent housing solutions. The outreach team comprises Outreach Specialists, Case Managers, Substance Abuse Counselors, Clinicians, and supervisory roles.

Key Responsibilities:

Direct Services (70%):
  • Deliver street and encampment-based case management to unhoused individuals in designated areas.
  • Facilitate referrals and connections to our internal team of Clinicians and Substance Abuse Counselors.
  • Coordinate services with community resources, including healthcare, benefits applications, employment counseling, and family reunification.
  • Collaborate with the Program Manager while maintaining accurate records of services rendered.
  • Engage with community partners to enhance support services for participants.
  • Utilize supervision effectively, ensuring open communication and updates on client intake activities.
  • Adhere to agency and program policies and procedures.
  • Report any suspected cases of abuse appropriately.
  • Maintain professional relationships and conduct.
  • Document all case management activities thoroughly and accurately as required by grant contracts.
  • Conduct client intake assessments as necessary.
  • Work with participants to create and implement individualized case management plans, providing support to achieve housing and personal objectives.
  • Meet regularly with participants based on their needs.
  • Provide support services using a Critical Time Intervention (CTI) model.
  • Arrange transportation for participants to shelters and critical appointments.
  • Offer weekend and after-hours support as needed.
  • Assist with additional duties as assigned.
Documentation (20%):
  • Record all case management activities in HMIS within two business days and maintain all relevant documentation.
  • Engage in ongoing program evaluation and alignment with industry best practices.
  • Continuously seek to enhance the quality and effectiveness of program services.
  • Conduct regular reassessments of client needs and document findings in HMIS.
General Duties (10%):
  • Attend all relevant meetings and training sessions.
  • Assist with office-related tasks as necessary.
  • Utilize supervision effectively, maintaining regular communication and updates on activities.
  • Follow agency and program policies and procedures diligently.
  • Report any suspected cases of abuse properly.
  • Perform other duties as assigned.
Minimum Qualifications:
  • Bachelor's degree with relevant experience in nonprofit organizations (or 2 years of case management experience in lieu of a degree).
  • Understanding and sensitivity to the needs of the unhoused population.
  • Experience in building relationships with community partners and external agencies.
Skills and Competencies:
