Position Summary
The BRC Case Management Specialist plays a vital role in supporting individuals experiencing homelessness. This position is based at the Boccardo Reception Center and is designed for those who are dedicated to making a difference in the lives of others. The role is temporary, with a focus on providing comprehensive support and resources to participants in need.
Program Overview:
HomeFirst operates various emergency shelter facilities throughout Santa Clara County, offering essential services such as nightly accommodations, meals, and access to hygiene facilities. Our shelters prioritize harm reduction and housing-first principles, ensuring that participants receive the support they need to transition to stable housing.
Key Responsibilities:
Direct Service (70%):
- Deliver case management services to individuals experiencing homelessness at designated locations.
- Coordinate with community resources to facilitate access to healthcare, benefits, employment counseling, and family reunification.
- Maintain accurate records of services provided and collaborate with the Program Manager.
- Engage with community partners to enhance support services for participants.
- Conduct client intakes and assessments as necessary.
- Develop and implement individualized case management plans to help participants achieve their goals.
- Provide transportation and support for critical appointments and services.
- Be available for weekend and after-hours support as needed.
- Document all case management activities in a timely manner.
- Participate in program evaluations and align services with best practices.
- Conduct ongoing assessments of client needs and document changes.
- Attend relevant meetings and training sessions.
- Assist with administrative tasks as required.
- Maintain open communication with team members and provide regular updates.
- Bachelor's degree or a minimum of three years of experience in human services.
- Bilingual in Spanish is preferred.
- Strong communication skills, both written and verbal.
- Ability to manage multiple priorities and meet deadlines.
- Understanding of the Housing First approach and comfort in low-barrier shelter settings.
