Overview

The Licensed Practical Nurse (LPN) operates under the supervision of a Registered Nurse or Nurse Manager, adhering to the LPN Scope of Practice. This role involves delivering comprehensive care tailored to the patient's needs, encompassing physical, emotional, developmental, psychosocial, and educational aspects. The LPN is accountable for the coordination and provision of exceptional care, which includes preparing patients for examinations, assisting with treatments, coordinating diagnostic tests, and documenting care in medical records. Additionally, the LPN plays a crucial role in patient and family education, participates in staff meetings, and engages in ongoing professional development. They may also serve as a mentor for newly onboarded LPN colleagues.

Key Responsibilities

Essential Functions include:
  • DIRECT PATIENT CARE: Evaluates and addresses the comprehensive needs of patients, following physician directives with RN oversight.
  • Routine Nursing Procedures: Executes standard nursing tasks under RN supervision to fulfill patient requirements.
  • Productivity Standards: Adheres to established productivity benchmarks.
  • Clinical Skills: Demonstrates proficiency in accessing and de-accessing ports, including line flushing and maintaining sterile techniques.
  • Medication Administration: Administers medications in accordance with defined protocols.
  • Training and Supervision: Seeks guidance from leadership as necessary.
  • Delegation: May assign tasks to unlicensed personnel, as appropriate.
  • Patient-Centered Care: Provides culturally sensitive care that aligns with the specific characteristics of the patient population.
  • Collaboration: Works closely with patients and their families to create a safe and comfortable care environment.

Coordination of Care:
  • Collaborates with RNs and other healthcare team members to ensure seamless care delivery.
  • Communicates changes in patient conditions to the appropriate healthcare professionals.
  • Partners with RNs to develop and adjust individualized care plans based on patient needs.

Patient Education:
  • Acts as an educator to empower patients and caregivers in self-care practices.
  • Utilizes teach-back methods to confirm understanding of self-care instructions.

Professional Development:
  • Engages in the orientation and training of new staff and nursing students.
  • Participates in continuing education and in-service training as required.
  • Maintains current knowledge in clinical practices relevant to patient care.

Communication and Documentation:
  • Ensures timely and accurate completion of all required documentation.
  • Applies regulatory guidelines appropriately in daily tasks.
  • Checks and responds to communications promptly.

Qualifications

Required Minimum Education:
  • Practical Nursing Diploma is mandatory.

Required Minimum Experience:

  • Previous experience in healthcare is preferred.
  • Experience in oncology or outpatient settings is advantageous.

Required Minimum Skills:

  • Demonstrates strong leadership, problem-solving, organizational, and time management abilities.
  • Exhibits proficient assessment and clinical skills.
  • Possesses excellent written and verbal communication skills.
  • Computer proficiency is essential.

Required Licenses and Certifications:

  • Licensed Practical Nurse certification is required.
  • Basic Life Support certification is required.
  • BLS Instructor certification is required.
  • BLS Provisional certification is required.
