Overview
The Licensed Practical Nurse (LPN) operates under the supervision of a Registered Nurse or Nurse Manager, adhering to the LPN Scope of Practice. The LPN is dedicated to delivering comprehensive care to patients, addressing their physical, emotional, developmental, psychosocial, and educational needs. This role involves the coordination and provision of high-quality care, including preparing patients for examinations, assisting during medical procedures, coordinating diagnostic tests, and documenting treatments in medical records. The LPN also plays a crucial role in patient and family education, participates in staff meetings, in-services, special projects, and ongoing education, and may serve as a mentor for newly hired LPN colleagues.
Responsibilities
Core Responsibilities and Essential Functions
- DIRECT PATIENT CARE: Assess and manage the total needs of patients following physician orders with RN oversight, utilizing the nursing process.
- Routine Nursing Procedures: Perform necessary nursing tasks under RN supervision to meet patient needs.
- Productivity Standards: Meet established productivity benchmarks.
- Technical Skills: Demonstrate proficiency in accessing and de-accessing ports, including flushing lines and maintaining sterile techniques.
- Medication Administration: Exhibit knowledge and skill in administering medications as defined within the scope for Wellstar Health System.
- Training and Supervision: Seek guidance and training from leadership as needed.
- Delegation: Appropriately delegate tasks to unlicensed staff within their scope of practice.
- Patient-Centered Care: Provide culturally sensitive, relationship-based care tailored to individual patient characteristics.
- Collaboration: Partner with patients, families, and significant others to create a comfortable and safe care environment, focusing on patient goals.
- COORDINATION OF SERVICES: Collaborate with RN and team members to coordinate care and services.
- Communication: Inform RN, physician, or clinical supervisor of any changes in patient condition.
- Plan of Care: Work with RN to develop and adjust individual care plans based on patient needs and physician orders.
- EDUCATION: Act as a patient educator to promote self-care and caregiver support.
- Teach Back Method: Utilize this method to ensure patient and caregiver understanding of self-care.
- STAFF EDUCATION: Participate in the orientation and training of new employees and nursing students.
- Continuing Education: Attend required continuing education and in-services.
- Clinical Knowledge: Maintain current knowledge in clinical practices for oncology patients.
- DOCUMENTATION: Ensure accurate and timely completion of required documentation.
- Regulatory Compliance: Apply knowledge of regulatory guidelines appropriately.
- Communication: Check voice mail and work email regularly, returning patient calls as necessary.
Required Minimum Education
- Practical Nursing Diploma Required.
- Preferred previous healthcare experience.
- Preferred experience in oncology, ambulatory, or outpatient settings.
- Strong leadership, problem-solving, organizational, and time management skills.
- Excellent assessment and clinical skills.
- Effective written and verbal communication skills.
- Proficient computer skills.
- Licensed Practical Nurse Required.
- Basic Life Support Required.
- BLS - Instructor Required.
- BLS - Provisional Required.
