Overview

The Licensed Practical Nurse (LPN) operates under the supervision of a Registered Nurse or Nurse Manager, adhering to the LPN Scope of Practice. The LPN is dedicated to delivering comprehensive care to patients, addressing their physical, emotional, developmental, psychosocial, and educational requirements. This role involves ensuring the coordination and provision of exceptional care, which includes preparing patients for examinations, assisting during procedures, coordinating diagnostic tests, and documenting treatments in medical records. The LPN also plays a vital role in educating patients and their families. Participation in staff meetings, in-services, special projects, and ongoing education is expected. Additionally, the LPN may serve as a mentor for newly hired colleagues.

Key Responsibilities

Essential Functions include:
  • DIRECT PATIENT CARE: Assess and address the comprehensive needs of patients following physician directives with RN oversight, utilizing the nursing process.
  • Execute routine nursing tasks to fulfill patient needs under RN supervision.
  • Meet established productivity benchmarks.
  • Exhibit proficiency in accessing and de-accessing ports, including flushing lines and maintaining sterile techniques.
  • Demonstrate competence in medication administration as per Wellstar Health System guidelines.
  • Seek guidance, training, or supervision from leadership as necessary.
  • Delegate tasks to unlicensed personnel when appropriate and within the scope of practice.
  • Deliver relationship-based, patient-centered care that respects individual characteristics (e.g., age, gender, health conditions) in a culturally sensitive manner.
  • Collaborate with patients, significant others, and family members to create a comfortable and safe care environment, prioritizing the patient's goals in the care plan.
  • COORDINATION OF SERVICES: Collaborate with RNs and other team members to coordinate care and services.
  • Notify RNs, physicians, or clinical supervisors of any changes in patient conditions as necessary.
  • Work with RNs to develop and adjust individualized care plans based on patient needs and physician orders.
  • PATIENT AND CAREGIVER EDUCATION: Act as an educator for patients to promote self-care and caregiver support.
  • Utilize teach-back methods to ensure understanding of self-care among patients and caregivers.
  • STAFF EDUCATION AND PROFESSIONAL DEVELOPMENT: Participate in the orientation and training of new staff and nursing students as appropriate.
  • Commit to ongoing education and in-services as required.
  • Maintain current clinical knowledge relevant to oncology patient care.
  • COMMUNICATION AND DOCUMENTATION: Ensure accurate and timely completion of required documentation.
  • Demonstrate knowledge and appropriate application of regulatory standards.
  • Check and respond to voice messages and emails daily as part of triage responsibilities.
Qualifications

Minimum Education Requirements:
  • Practical Nursing Diploma is required.
Minimum Experience Requirements:

  • Previous healthcare experience is preferred.
  • Experience in oncology or outpatient settings is preferred.
Minimum Skills Required:

  • Strong leadership, problem-solving, organizational, and time management skills.
  • Excellent assessment and clinical skills.
  • Effective written and verbal communication abilities.
  • Proficient computer skills.
Licenses and Certifications Required:

  • Licensed Practical Nurse certification is required.
  • Basic Life Support certification is required.
  • BLS Instructor certification is required.
  • BLS Provisional certification is required.
