POSITION SUMMARY: This role is crucial in the formulation, execution, oversight, and adjustment of Individual Service Plans/Plans of Care. The individual will facilitate connections with relevant community resources and track progress towards set objectives. Additionally, the role involves ensuring compliance with documentation standards, timelines, and applicable regulations.


KEY RESPONSIBILITIES:

  • Oversee the recruitment, assessment, guidance, and termination of staff.
  • Ensure comprehensive training for all personnel.
  • Address all staff and client-related matters.
  • Act as a liaison for client concerns.
  • Report issues related to workplace injuries, family leave, and incidents.
  • Conduct site evaluations.
  • Supervise Lead Staff in maintaining facilities and vehicles in accordance with industry standards.
  • Review progress notes, respond to audits, verify timecards, and submit payroll adjustments.
  • Ensure all positions are adequately filled.
  • Monitor overtime and compliance with approved schedules and pay rates.
  • Manage emergency situations as they arise.
  • Report any instances of abuse or neglect.
  • Maintain comprehensive client case files.
  • Ensure ongoing family communication.
  • Track behavioral incidents and medication errors, providing follow-up as necessary.
  • Complete incident reports following established procedures.
  • Monitor and address client behaviors in accordance with their behavior management plans.
  • Coordinate community outings and activities for clients.
  • Report deficiencies related to the Plan of Care.
  • Be available for on-call duties and conduct quality control checks as needed.
  • Facilitate staff and client meetings.
  • Ensure adherence to all compliance standards.
  • Prepare weekly reports for relevant stakeholders.
  • Compile quarterly reports and respond to collected data appropriately.
  • Conduct regular audits as assigned.
  • Ensure all services are billed and approved prior to clocking out.
  • Maintain current utilization tracking and documentation.
  • Collaborate with PASSE staff to support clients.
  • Ensure all necessary client documentation is completed and filed correctly.
  • Keep calendars and communications up to date.
  • Drive clients on outings as needed, maintaining safe driving practices.
  • This position is classified as safety-sensitive.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE:

A Bachelor's degree in a human services-related field is preferred, or two years of advanced education in human services with two years of relevant experience. Alternatively, four years of experience in a related field may substitute for formal education.

Two years of verifiable experience with individuals with developmental disabilities is required, with mentorship from a certified case manager during the initial two years of employment.


LANGUAGE SKILLS:

Ability to read and interpret business documents, professional journals, and governmental regulations. Proficient in writing reports and correspondence, and effectively communicating with groups.


MATHEMATICAL SKILLS:

Ability to perform calculations related to discounts, interest, and other financial figures.


REASONING ABILITY:

Ability to solve practical problems and interpret various instructions.


PHYSICAL DEMANDS:

Regularly required to use hands, stand, walk, and reach. Occasional lifting of up to 25 pounds may be necessary.


QUALITY ASSURANCE & COMPLIANCE: Adherence to quality assurance and HIPAA procedures is mandatory, including maintaining accurate consumer records.
