POSITION SUMMARY: The Service Manager plays a crucial role in the creation, execution, oversight, and adjustment of Individual Service Plans/Plans of Care. This position is responsible for connecting clients with suitable community resources and tracking progress towards set goals. Additionally, the Service Manager ensures that all documentation adheres to required timelines, licensing standards, and relevant laws and regulations.


KEY RESPONSIBILITIES:

  • Oversee the recruitment, evaluation, guidance, and termination of staff.
  • Ensure comprehensive training for all staff members.
  • Address all staff and client-related concerns.
  • Act as a liaison for client-related issues.
  • Report any incidents related to Workers Compensation, FMLA, and other relevant matters.
  • Conduct site visits as necessary.
  • Supervise Lead Staff in maintaining compliance with FCC, DDS, CARF, and PASSE standards.
  • Review progress notes, respond to audits, and manage payroll documentation.
  • Ensure all positions are adequately filled.
  • Monitor staff overtime and compliance with approved schedules and pay rates.
  • Manage emergency situations as they arise.
  • Report any instances of abuse or neglect.
  • Maintain confidentiality of client case files and family communications.
  • Monitor behavior incidents and follow up as required.
  • Coordinate community outings and activities for clients.
  • Ensure compliance with CARF, DDS, and PASSE requirements.
  • Complete weekly and quarterly reports for relevant departments.
  • Conduct regular audits to ensure quality control.
  • Maintain accurate documentation in the EHR/HR platform.
  • Drive clients to various locations as needed, requiring a valid driver's license and insurance.
  • This position is considered safety-sensitive.

QUALIFICATIONS:

To be successful in this role, candidates must meet the following criteria:

  • Bachelor's degree in a human services-related field or equivalent experience.
  • Two years of advanced education in human services plus two years of case management experience, or four years of relevant experience may substitute for education.
  • Two years of verifiable experience with individuals with developmental disabilities, mentored by a certified case manager during the first two years of employment.

SKILLS:

Strong analytical and communication skills are essential. The ability to interpret business documents, write reports, and effectively communicate with groups is required.
