Job Summary

We are seeking a dedicated and experienced professional to join our team at HOMES FOR THE HOMELESS, INC as a Case Manager. This role will involve working with families in need to provide essential services and resources to improve their quality of life.

Key Responsibilities

  • Manage and update records for families, ensuring accurate and timely documentation
  • Develop and implement service plans based on client needs, working closely with families to set and achieve goals
  • Refer families to external agencies for various services, including healthcare, education, and employment support
  • Document client progress regularly, using data to inform service delivery and improve outcomes
  • Assist families in finding permanent housing, working with landlords and property managers to secure suitable accommodations
  • Ensure compliance with documentation guidelines, maintaining accurate and up-to-date records
  • Conduct unit inspections to ensure a safe and healthy living environment

Requirements

  • Minimum of a Bachelor's or Associate's Degree in a relevant field, such as social work or human services
  • Experience in case management or a related field, with a proven track record of success
  • Proficiency in Microsoft Office, including Outlook, Word, and Excel
  • Ability to speak multiple languages is beneficial, but not required
