About the Role:

At Breaking Ground, we are committed to the belief that everyone deserves a place to call home. Our organization manages nearly 4,000 housing units throughout New York City, as well as in upstate New York and Connecticut. We employ a supportive housing model that integrates affordable housing with essential services, effectively addressing chronic homelessness.

Our mission extends beyond merely providing shelter. We implement innovative programs aimed at assisting individuals facing street homelessness, particularly those who have endured prolonged periods without stable housing. Through initiatives such as our Street to Home outreach program and transitional housing resources, we facilitate the journey of individuals towards securing a permanent residence.

The Street to Home program at Breaking Ground maintains ongoing communication with over 2,000 individuals experiencing homelessness each year, successfully connecting more than 500 of them to housing, medical care, mental health services, substance abuse counseling, and other critical supports. The tailored assistance provided at each stage of their journey ensures that over 95% of individuals placed by Breaking Ground achieve stable housing.

Key Responsibilities:

  • Deliver direct services to a caseload of approximately 25 clients, including assessments, benefits assistance, and other essential support to aid clients in reaching their housing and recovery objectives.
  • Conduct psychosocial assessments.
  • Facilitate connections to psychiatric services.
  • Link clients to medical and substance use treatment facilities as necessary.
  • Assist clients in acquiring identification, public benefits, and other income sources.
  • Hold weekly meetings with clients to evaluate needs, promote harm reduction, and develop service plans.
  • Prepare all necessary documentation for housing applications.

Provide guidance and support to clients in securing permanent housing.

  • Maintain precise record-keeping in accordance with agency, City, and State standards.
  • Intervene during crisis situations.
  • Accompany clients to appointments as required.
  • Upon completion of training, assist clients with self-administration of medication following Breaking Ground's protocols.
  • Perform other assigned tasks as needed.

Minimum Qualifications:

  • Bachelor's Degree or equivalent professional experience.
  • Two years of experience working with homeless adults and substance abuse issues is preferred.
  • Familiarity with harm reduction and motivational interviewing techniques is highly valued.
  • A valid NY or NJ driver's license with a clean driving record is required.
  • Proficiency in Microsoft Word and Excel, along with the ability to learn new software, and excellent written and verbal communication skills.
  • Bilingual proficiency in Spanish and English is a plus.
