Youth Opportunity Program Manager

We are seeking a highly motivated and experienced Case Manager to join our team at Commonpoint Queens. The successful candidate will be responsible for managing a caseload of opportunity youth, providing individual and group support, and facilitating wraparound services to promote academic and career success.
Key Responsibilities- Case Management: Manage a caseload of 30-35 participants, conducting intake assessments, and assisting with eligibility documentation collection.
- Program Implementation: Partner with fellow case managers to provide workshops, retention engagement activities, and cohort celebrations.
- Participant Support: Provide individual and group mental health support, monitor participant program performance and attendance, and provide detailed case notes and data entry.
- Collaboration and Communication: Maintain effective communication and rapport with participants, staff, and support systems, and conduct care coordination with outside treatment providers/referrals.
- Education: Bachelor's degree in Human Services, Psychology, or related field.
- Experience: 3+ years of case management experience with opportunity youth.
- Skills: Trained in trauma-informed care, crisis intervention, and de-escalation, with experience working with MS Office, Google applications, and PDF.
- Language: Bilingual or multilingual is highly preferred, preferably in Spanish, Haitian Creole, or Bengali.
- Location: Commonpoint Queens, 82-17 Parsons Blvd.
- Schedule: Monday-Friday, 9am-5pm.
- Salary: $50,000 (BA/BS) - $55,000 (MA, MSW, and/or advanced experience).
