- Service Coordination: Coordinate services with Housing Specialists, other staff members, and external partners to ensure clients receive comprehensive support.
- Advocacy and Support: Provide advocacy and support to clients to help them access entitlements and services, and to strengthen their ability to make informed life decisions.
- Crisis Intervention and Counseling: Provide crisis intervention and counseling services to clients as needed, and to help them develop coping strategies and skills.
- Documentation and Reporting: Maintain accurate and up-to-date records of client interactions and service delivery, and provide regular reports to management as required.
- Collaboration and Communication: Work collaboratively with other staff members and external partners to ensure seamless service delivery and to promote a culture of teamwork and respect.
- Education and Experience: Bachelor's degree in Social Work or a related field, and at least 1 year of experience in case management or a related field.
- Skills and Abilities: Strong relationship-building skills, excellent organizational and time management skills, and the ability to work independently and as part of a team.
- Language Skills: Bilingual skills are an asset.
