Job Overview

Position: Case Manager

Location: Various opportunities available

Schedule: 8:00 AM - 5:00 PM, Monday through Friday

Compensation: $24/hour

Position Summary:

LHH Recruitment Solutions is seeking a dedicated Case Manager to assist individuals experiencing homelessness, particularly those facing challenges related to substance use and mental health. This role entails evaluating clients, formulating individualized case strategies, and tracking their progress within a supportive environment. Collaboration with probation officers, social workers, and family members is essential, along with maintaining comprehensive client documentation.

Key Responsibilities:

  • Oversee a caseload of 20-25 clients weekly, averaging 4-5 clients daily.
  • Perform daily wellness checks and assist with essential tasks such as laundry and room organization.
  • Lead group sessions and ensure adherence to daily schedules.
  • Participate in training and case reviews with funding organizations.
  • Manage crisis situations and de-escalate conflicts as they occur.
  • Support clients in securing housing and accompany them to view potential living spaces.
  • Conduct extensive administrative duties, including documenting client interactions and entering data into a statewide system.
  • Foster positive relationships with colleagues and community partners.

Qualifications:

  • Bachelor's degree in a relevant discipline.
  • Possession of a valid driver's license.
  • A minimum of 1 year of experience in case management.
  • Experience working with homeless populations, substance abuse issues, and mental health challenges.
  • Capability to manage diverse responsibilities and adapt to a dynamic work environment.
