Position: Case Manager - TLS Families Department: HOPICS

Classification: Non-Exempt, Full-time Supervisor: Program Manager

Compensation: $22.00 to $24.00 per hour

Overview

Under the supervision of the Program Manager, the Case Manager for Time Limited Subsidy is tasked with the crucial role of assessing eligibility, coordinating services, and triaging families and individuals seeking assistance through the Coordinated Entry System.

Key Responsibilities

  • Collaborate with project staff and partners to deliver housing placement and supportive services, ensuring clients transition successfully to permanent housing and maintain stability.
  • Manage a caseload of families or individuals, maintaining a ratio of 1:35.
  • Conduct monthly follow-ups with clients to monitor progress and provide necessary support.
  • Document participant records in HMIS upon eligibility determination and enrollment.
  • Update clients' housing status within 24 hours of any changes.
  • Track services, referrals, and complete case notes promptly following service provision.
  • Conduct Status Assessment Updates for significant changes in client circumstances within 24 hours.
  • Enter all client contact data into Clarity within 24 hours of meetings.
  • Ensure timely completion of client exit documentation following exit interviews.
  • Update program entry, status assessment, and exit pages for clients who have transitioned to permanent housing.
  • Maintain detailed and accurate case notes in DAP format within 24 hours.
  • Address data errors promptly as reported on a weekly basis.
  • Submit daily deliverables and weekly productivity reports to supervisors.
  • Assist clients in securing housing by collaborating with the Housing Team and attending landlord interviews.
  • Develop individualized housing plans and budgets with clients to ensure sustainability post-subsidy.
  • Prepare and upload Monthly Summary Reports into CLARITY.
  • Participate in case conference meetings regularly.
  • Verify eligibility documents for program enrollment and financial assistance requests.
  • Coordinate with various agencies to provide comprehensive services for housing stability.
  • Maintain accurate and up-to-date documentation in client files for effective case management.
  • Provide necessary client information and assessment outcomes to supervisors.
  • Prepare project reports in compliance with funding requirements.
  • Assist in training and supporting staff and partners involved in the project.
  • Conduct background checks on applicants and household members as required.
  • Adhere to confidentiality standards and maintain compliance with HIPAA and other regulations.
  • Represent the agency professionally at community events and meetings.
  • Build trusting relationships with clients through integrity and responsibility.
  • Engage with external agencies for potential long-term housing solutions.
  • Conduct regular field visits to clients' homes as necessary.
  • Maintain consistent attendance and complete additional duties as assigned.
  • Availability for evening hours on select days is required.

Qualifications

  • An associate degree in social services or a related field, with 1-2 years of relevant experience working with homeless populations, or two years of experience in the social service sector.
  • Preferred experience in case management and homeless programs.
  • For those in recovery, a minimum of three years of sobriety is required.
  • Proficient in Microsoft Office and database management.
  • Strong understanding of resource development and case management practices.
  • Ability to work effectively with diverse populations.
  • Excellent written and verbal communication skills.
  • Verification of employment eligibility and background clearance is mandatory.
  • Annual TB testing and CPR/First Aid certification are required.
  • Reliable transportation is essential.

Working Conditions

This role involves working in client-friendly environments, requiring travel to various shelters and access centers. The position may involve exposure to noisy environments.

Physical and Mental Requirements

The Case Manager will engage in various activities including sitting, standing, typing, and driving. The role demands the ability to manage distractions and adapt to changing priorities.
