Position Overview

The Community Support Coordinator is responsible for engaging with homeless individuals facing serious mental health challenges. This role focuses on facilitating access to essential psychiatric care, housing solutions, medical assistance, and various social services.

Key Responsibilities

  1. Conduct outreach initiatives to connect with homeless individuals, building rapport and trust.
  2. Perform comprehensive assessments to evaluate functional limitations and determine eligibility for services.
  3. Develop individualized Action Plans in collaboration with clients, ensuring regular updates as needed.
  4. Facilitate connections to vital services such as psychiatric treatment, housing, and vocational training.
  5. Provide direct assistance, including transportation, to help clients access necessary resources.
  6. Coordinate with other service providers to ensure continuity of care and comprehensive support.
  7. Engage with family members or significant others to promote stability and effective service implementation.
  8. Monitor client progress and satisfaction, adjusting service delivery as clinically indicated.
  9. Maintain accurate and timely documentation in compliance with agency guidelines and regulatory standards.
  10. Participate in staff meetings, training sessions, and case reviews to enhance service delivery.

Qualifications

Applicants should possess a Bachelor's degree in a human services field, along with two years of relevant experience in mental health and homelessness support. A valid driver's license and a clean driving record are required for those using personal or agency vehicles in the performance of their duties.
