Job Responsibilities:

Key Duties Include:

  • Conducting thorough assessments of patients' physical and emotional needs.
  • Formulating and implementing individualized care plans based on patient assessments.
  • Collaborating with multidisciplinary teams to ensure comprehensive patient care.
  • Monitoring patient progress and adjusting care plans as necessary.
  • Documenting all aspects of patient care accurately and in a timely manner.

Work Environment:
This position is for a full-time role on the night shift, requiring three 12-hour shifts per week. You will be part of a supportive team dedicated to professional growth and excellence in patient care.
