Support Coordinator

About the Role: The Support Coordinator collaborates closely with individuals, their families, advocates, and various agencies to oversee the development and execution of personalized plans.
Key Responsibilities:
- Take ownership of creating and implementing Individual Plans and Service Funding Plans for assigned individuals.
- Ensure the effective execution of recommendations from Individual Plan meetings.
- Facilitate and lead planning meetings, including regular reviews.
- Draft measurable and relevant training objectives for individuals served.
- Work collaboratively to form a team that is both effective for the planning process and preferred by the individual.
- Coordinate timely communication among families, staff, advocates, and agencies.
- Identify and arrange necessary care, including medical and emergency services, as well as daily transportation and additional resources.
- Maintain accurate and timely documentation as required.
- Adhere to all legal and agency policy requirements in performing duties.
- Undertake additional responsibilities and special projects as needed.
- A Bachelor’s Degree from an accredited institution, preferably in Human Services or a related field.
- At least one year of experience in developing and implementing Individual Plans for individuals with developmental disabilities, along with knowledge of health issues and first aid.
- Ability to build and maintain effective working relationships.
