Job Summary

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a vital role in facilitating the planning and organization of patient care activities, working closely with our healthcare professionals to ensure seamless care delivery.

Key Responsibilities

  • Coordinate patient care activities, including scheduling appointments, managing referrals, and communicating with healthcare providers.
  • Assist patients and their families with navigating the healthcare system, providing education and support as needed.
  • Collaborate with the care team to develop and implement individualized care plans, ensuring patient needs are met and goals are achieved.
  • Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with regulatory requirements.
  • Provide exceptional customer service, responding to patient inquiries and concerns in a timely and professional manner.
  • Participate in quality improvement initiatives, contributing to the development and implementation of process improvements.

Requirements

  • High school diploma or equivalent required; associate's or bachelor's degree in a healthcare-related field preferred.
  • At least two years of experience in a healthcare setting, preferably in a patient care coordination or clinical support role.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.

Preferred Qualifications

  • Experience working in a medical office or clinical setting, preferably in a patient care coordination or care management role.
  • Knowledge of healthcare regulations and compliance requirements, including HIPAA and OSHA.
  • Experience with electronic health records (EHRs) and other healthcare software systems.
